چکیده

Since the mid-1970s, the use of local anaesthesia by podiatrists has been a routine part of practice and has facilitated the application of many techniques that have been developed to the benefit of the patient. The methods employed range from local infiltration of the anaesthetic agent at the site of the lesion to nerve trunk block techniques that prevent sensory stimulus from the area of the foot the nerve supplies. The choice of method and site of injection depend on factors that are in the best interests of the patient and allow the lowest dosage of anaesthetic agent to be used. A single site on the weight-bearing plantar surface will generally involve blocking the tibial nerve, whereas a single site on the dorsum can be anaesthetized by local infiltration. However, techniques can be employed whereby local infiltration is used on the plantar surface or is delivered by injecting towards the plantar between the metatarsal heads. Multiple sites on the dorsum may be better anaesthetized by a block of the nerve that supplies the area, which could be the sural, saphenous, deep or superficial peroneal. If both of the latter are to be blocked, it is better practice to block the common peroneal nerve, but it should be remembered that this might also affect motor function in the leg even if the total dosage of the agent used is closely controlled and kept to a minimum. Study of the Intraplantar Injection of Lidocaine and Morphine on Pain Perception and the Influence of Morphine Dependence and Withdrawal on Lidocaine-Induced Analgesia in Rats

Background: Morphine and lidocaine are known to influence the perception of pain. The present study sought to determine the influence of local administration of morphine on lidocaine-induced analgesia in morphine non-dependent (MND), morphine dependent (MD) and morphine withdrawal (MW) animals. Methods: Adult male Wistar rats were divided into four groups: Control, MND, MD and MW rats. Lidocai...

Analgesia and anesthesia during IUD insertion: local anesthetics

Background: Intrauterine Device (IUD) is an effective and long term method of contraception that can be used without continued effort by the user to prevent pregnancy perfectly. Pain during IUD insertion is one of the obstacles to properly using the device. Thus, using different methods of analgesia and anesthesia to alleviate this pain has been studied by many researchers. This review article ...

Wound infiltration with local anaesthetics in ambulatory surgery.

PURPOSE OF REVIEW Wound infiltration analgesia using local anaesthetics has been used for several decades. Recently, newer techniques to prolong analgesia have developed, including the use of catheters and injection of local anaesthetics or other adjuvants, and local infiltration analgesia using large volumes of local anaesthetics injected into different tissue planes. The aim of this review is...

Mode of delivery following labor epidural analgesia: influence of ropivacaine and bupivacaine.

Epidural analgesia is a popular and effective method for pain relief during labor. Bupivacaine is a commonly used local anesthetic for labor epidural analgesia. Ropivacaine is an amino acid local anesthetic that is structurally related to bupivacaine with a similar potency and duration, but ropivacaine has less cardiac toxicity than bupivacaine and produces less motor blockade. These properties...

Progress in analgesia for labor: focus on neuraxial blocks

Neuraxial analgesia is widely accepted as the most effective and the least depressant method of providing pain relief in labor. Over the last several decades neuraxial labor analgesia techniques and medications have progressed to the point now where they provide high quality pain relief with minimal side effects to both the mother and the fetus while maximizing the maternal autonomy possible fo...

Postoperative pain relief using intermittent intrapleural analgesia following thoracoscopic anterior correction for progressive adolescent idiopathic scoliosis

BACKGROUND Thoracoscopic anterior scoliosis instrumentation is a safe and viable surgical option for corrective fusion of progressive adolescent idiopathic scoliosis (AIS) and has been performed at our centre on 205 patients since 2000. However, there is a paucity of literature reporting on or examining optimum methods of analgesia following this type of surgery. A retrospective study was desig...
